Documents - Add Forms

1. Click the Documents button from the Data Entry Screen.



2. You will have two buttons at the bottom to add Forms or Documents.




3. Click Add Forms buttonA list of forms will populate for you to add to your entry and fill out.



 Click to choose the forms and then click OK.





4Fill out the Forms that were pulled in. Click the form, enter the necessary information. Once complete, click Save Form.


(Note: Red fields in the form are required, you must fill out this information. If you're done with the forms, you can also click Transfer Form and it will transfer the document to the FMP Office personnel.)




It will now look like the below screenshot.
